What it does

How to use it

  1. Choose the event type: incident, near miss, unsafe act, unsafe condition, first aid or lost-time injury.
  2. Fill in what happened, where, when and who was involved.
  3. Score severity and likelihood on the risk matrix to get a risk rating.
  4. Add corrective and preventive actions with an owner and due date, then save to the register.

Frequently asked questions

What is the difference between an incident and a near miss?

An incident caused actual harm, damage or loss. A near miss had the same potential but no consequence, only because of timing or luck. Near misses are the more valuable report: they reveal the same hazard at no cost, which is why mature sites record many more of them than incidents.

Why score potential severity rather than actual?

Because the actual outcome is often decided by chance. A spanner dropped from height that hits nobody is a near miss with a potential severity of fatality. Scoring potential keeps the control effort proportionate to the hazard rather than to the luck.

What makes a good corrective action?

One that removes or reduces the hazard rather than relying on people being careful. Follow the hierarchy of control: eliminate, substitute, engineer, administrate, then personal protective equipment. An action that reads 'counselled the worker' usually leaves the hazard untouched.

Is this a statutory reporting tool?

No. It is an internal register that helps you capture events consistently and track actions to closure. Statutory notification under the Factories Act, state rules or the relevant regulator remains your obligation, with their own forms and timelines.

Where is the register stored?

It is saved in this browser and synced to your account on the CRMTECH server, so it is restored on any device you sign in from, with earlier versions kept. Every event is also recorded in the server audit log for this application. Export to CSV or JSON for statutory records.

Is EHS Incident & Observation Log free?

Yes. EHS Incident & Observation Log is free for everyone and needs no sign-up to use. An account is only needed to export, download or print, because every export is recorded.

Are my files uploaded to a server?

No. EHS Incident & Observation Log works inside your web browser, so the files you open are processed on your own device. If you sign in, the work you save (such as drafts and registers) is also kept in your account so it follows you to other devices.

Does it work on a phone?

Yes. It works in current versions of Chrome, Edge, Safari and Firefox on phones, tablets and computers, with nothing to install.
